Please ensure each section is completed, as nominations will be assessed against all selection criteria. Provide clear examples and supporting evidence wherever possible to help the judging panel understand the nominee’s contribution, achievements and impact in advancing sustainable healthcare.

Describe how the nominee or team has demonstrated leadership or innovation in embedding sustainable healthcare practices. Include examples of influence on services, policy, culture or broader system change.
Describe how the nominee or team has engaged others, advocated for sustainable healthcare or shared knowledge and learnings. Include examples of collaboration or broader influence beyond their immediate service.
